GCLA DUES MEMBERSHIPS ARE $100.00 PER YEAR. AND A COMPANY DUES MEMBERSHIP FOR ALL EMPLOYEES IS $200.00 PER YEAR WITH A BACKGROUND CHECK FEE OF $50.00 PER APPLICATION. $50.00 BACKGROUND FEE WILL BE WAIVED FOR APPLICANT'S HOLDING A STATE LOCKSMTIH LICENSE OR PERC CARD.

ALL NEW APPLICANTS (EXCEPT ASSOCIATE MEMBERS) ARE REQUIRED TO HAVE A CURRENT STATE LOCKSMITH LICENSE OR PERC CARD WITHIN SIX (6) MONTHS OF JOINING THE GCLA. FAILURE TO ACQUIRE A STATE LOCKSMITH LICENSE OR PERC CARD IS GROUNDS FOR DISMISSAL.

This application must be accompanied by A $50.00 initiation fee (returned if application is not approved), current photograph, and set of fingerprints (this may be secured at any police department), business card, letterhead, or phone book ads.

All questions must be answered fully. Failure to answer all questions or any misrepresentation by omission or concealment by misleading, false or partial answers may serve as basis for denial of membership. All information will be held confidential.

I hereby authorize the GCLA to conduct such inquiries of my application for membership as may be necessary in the Association's discretion. I understand that this information is to be used solely for membership application and will not be disseminated to any other agency.

A photocopy or digital reproduction of this release is considered valid as the original but is only effective for the limited purpose cited in this release.

1. Be notified when information obtained is used to deny membership or make any membership decision.

2. Be informed of the nature and substance of the information obtained; and

3. Dispute and correct inaccurate or incomplete information.

I understand that this release constitutes a legal document. I understand that I should consult with an attorney if there are any provisions that I do not understand.
